Moving on from Heatherwood Court - "Pathways to Independence"

From the moment a client walks through the doors at Heatherwood Court our primary goal is to start to equip them with the skills required to live a more independent life. The MDT will be focused on delivering a treatment plan with the goals of amelioration of symptoms, reduction of distress and problematic behaviours. However the ultimate goal is to rehabilitate the client into the least restrictive community based placement. This seamless service is an important part of our 'model of care' as the clinicians will operate over both the hospital and community homes environments to facilitate continuity of care. This stability of the clinicians across environments has proven highly beneficial for clients in our other Group Services. Our experience is that this maximises the prospect of the transition being successful for the client.

Ludlow Street Healthcare Group continues to develop off-site satellite units that provide stepping stones towards greater independent and community based living. These ' real home' units offer intensive support for clients with a range of complex needs with a team of experienced and highly motivated professionals. Ongoing psychological treatments can then be continued by the professional team who delivered these in Heatherwood.

The clients are also encouraged and facilitated to become involved in community based rehabilitation and education through local colleges and services. This not only enhances their level of skills but leads them closer to the goal of independence.
